All together now: how connected digital systems support connections in care communities

Personal connections shape care quality. In the world of social care, where human connections and digital integrations are increasingly intertwined, standalone systems can leave care professionals, and the people they support, feeling similarly isolated. 

A lesson Community Integrated Care (CIC) learned first-hand when starting their digitisation journey. They introduced several separate systems to help them improve their care quality. However, the inability of these systems to effectively share information led to loose ends, duplication and time-consuming back tracking.  

CIC addressed this by partnering with Nourish Care to build an integrated digital ecosystem. By connecting functionality, they were able to improve care quality intuitively. With more informed care teams and more engaged clients underpinned by a more accessible system. 

Their ability to fluidly link their incident management and care management enabled them to provide comprehensive care journeys, shaped to and by the unique experiences of each person utilising their service.
